5 yr weight history...ugh

Okay, so I had my consult with dr. yesterday, and he said that because of my diabetes that HE WOULD approve me for the surgery. My BMI was 34.6.. grrr... anyhoot. The nurse told me that my insurance required a 5 year weight history report. So, I have to go to my drs. that I've seen in the past 5 yrs and get how much I weighed. My question is this... how many reports will they need during thos 5 yrs. Like, is there a certain amount of times during those 5 yrs that I should have been weighed when I went to the dr.? Does this question make sense? I think I am confusing myself now... so sorry. Thanks anyway!!

Normally each time you go to the Doctor they direct you to the scale first thing. So all that data should be in your chart. Each insurance policy has its own requirements, so it would be difficult to be precise but with my policy they accepted a list of dates and weights written out from my PCP. The best way to tackle it is call your insurance provider give them your policy number and ask them to send it to you in writing. Good luck! C'ya Bob

I dont know what insurance you have, but I have United Healthcare and they required the same thing. Collecting Medical records with my weight since 2004 was the hardest thing, i know everytime I have been anywhere they weight me, but no one has the information when i need it . (I decided from now on I am going to decline since it is not kept for my use anyway. :) ) But I was told that I could send in photos with the year and approximate weight from the last 5 years if i could not get medical records. Also, you know the records where there is no weight but they put the lovely note " Severly Obese"? Well that is acceptable with the photos.

I have CIGNA and I had to have a weight history, I just called my former PCP, and when I had missed a year with him, I called my gyno, she had one for all the years cause of our lovely yearly checkups!   It wasn't a big deal just a couple of phone calls on my part.
